Insurance
If you have dental insurance please bring a form. We need you to fill out the subscriber and patient areas on the form, we’ll take care of the rest. Please bring any co-pay or deductibles you’re aware of. We are willing to make financial arrangements if necessary, however we do accept cash, checks, and credit cards for your convenience.
We know dental insurance can be hard to understand. If you are unsure whether your insurance will cover your procedures, please give us a call! We can help you figure out what is and is not covered and we can create a dental plan around your individual insurance benefits.
