East Avenue Dentistry Savings Plan helps patients and families without dental benefits get access to quality, trusted dental care. The East Avenue Dentistry Savings Plan is designed to help our patients and their families save money on periodic and preventive dental treatment and services.
ANNUAL MEMBERSHIP FEES THROUGH DECEMBER 31st, 2024
INDIVIDUAL PLAN $348.00
PEDIATRIC (CHILDREN 0-18) PLAN $315.00
TWO PEOPLE IN ONE HOUSE $558.00
FAMILY PLAN $685.00
(Up to five family members maximum. Can include self, other and children under 21 years of age residing in the same household.)
(10% Service fee will be added if patient wants to pay monthly though Carecredit®)
ANNUAL MEMBERSHIP INCLUDES:
· Two routine cleanings, exams and fluoride treatments (for patients up to age 18) for each person per membership year. *Please see exclusions below*
· One set of full x-rays or Panorex and one set of bite-wing x-rays for each person per membership year.
· 10% discount for most other dental care for each person per membership year.
· Unlimited periapical radiographs as needed for diagnosis.
· Unlimited Intraoral photos as needed for diagnosis.
With your membership, you can save more than $100 on routine cleanings, exams and x-rays. Plus, 10% off most other dental services offered at East Avenue Dentistry PLLC.
*Comprehensive hygiene services such as Scaling and Root Planing, Debridement, and Periodontal Maintenance are NOT included in the routine cleaning benefit. Please ask the front desk or your provider if you have questions regarding the DSP policy for these services.
TERMS AND CONDITIONS:
It is important to us that you understand the terms and conditions of the East Avenue Dentistry Savings Plan so we are not hiding it in "fine print." Payment for service is expected at the time it is rendered in order to receive your savings unless previous financial arrangements are made. Fees may be paid by cash, check, credit or FSA/HSA cards. East Avenue Dentistry Savings Plan is not dental insurance and does not serve as a substitute for comprehensive dental insurance coverage. Participation will terminate automatically if the patient obtains dental insurance and uses it at East Avenue Dentistry PLLC. Discounts may not be used in conjunction with any other discount plans or insurance programs. This plan does not cover expenses incurred from Workers Compensation claims related to injuries or trauma-related injuries such as auto accidents. East Avenue Dentistry reserves the right to make changes to these terms and conditions at any time. Once membership is purchased, fees are non-refundable. Membership period is valid for one year from date of enrollment. Invisalign, dental veneers and teeth whitening are not included. Membership is only valid at East Avenue Dentistry. There are no refunds if patients obtain insurance during the year or do not use their East Avenue Dentistry Saving Plan benefits. Further restrictions may apply.
